IVHS & S.P.C.A. uses Authorize.net to process the Pet License form payment. There will be a small fee of $2.00 added to your license cost. Required fields are denoted by an asterisk *. In order to complete your license application, IVHS & S.P.C.A. requires a current rabies certificate and proof of alteration (if applying for the altered pet price). Applications will not be processed without this information.
Note: Veterinary receipts for vaccination will not be accepted as proof of vaccination and will delay the license application process. For the senior discount, proof of age is required. After completing the online form and submitting a payment through Authorize.net please mail or fax the appropriate documentation to IVHS, Licensing Department.
